Why Iron? Why Us???

Did I mention my mentees ask the meanest (!) & most meaningful questions?  Especially those early career practitioners engaged in our New Grads program! And it’s been almost a whole month minute since I’ve talked about iron, so I wanted to share this gem from Katherine Geary:

“Why are so many women seriously low in iron? Periods aren’t a modern invention and it seems a bit of a design fault to have half the population dragging themselves around with iron deficiency anaemia! So why is it so common, even in women who seem to have reasonable dietary intake and low/average menstrual blood loss?”

Well I am so glad you asked!  And not because I or anyone has the definitive answer.  But there are some excellent voices joining in this conversation about how iron nutriture has seemingly become the collective female Achilles and how, in fact, this might be, as opposed to a ‘design fault’…wait for it…an evolutionary advantage!  Particularly hard to believe I bet for those of us ‘dragging ourselves around with iron deficiency anaemia’…to contemplate that this makes you, in fact, some kind of Superhero! But let’s back up there a bit.  Firstly, in light of iron being the 4th most common element on earth and concurrently the number one micronutrient globally we struggle to stay replete in…the contradiction is striking.  Add to this the fact that while women and children of developing countries experience higher rates of deficiency & IDA, those of us living the ‘good life’ in the top pastures of the developed ones, are still affected at strikingly high rates.  Katherine’s right of course, periods are not new…so have the rates of iron deficiency always been so high, or if not, what’s changed?  Well, a few things.

As so often seems to be the case 🙄  most of the finger-pointing is at the problem child period: the Agricultural Revolution. This radical shift in diet, away from meat towards cereals, is a clear contributor.

And then came the infections. Both the GIT ones, with bacteria (not naming any names, H.pylori!) parasites & helminths etc getting cosy and cohabiting inside no-longer nomadic humans (stealing even more of our precious iron) and  then following on as well, the rise of infectious illnesses,  flu, smallpox, malaria, TB, etc. [Ok this is the bit where you can don your cape!] If you have one of the latter, then having one of the former can be really handy!  Let’s say this another way.  Iron deficiency is considered protective against  the infections that can kill.

That’s why we see that characteristic  sequestering of iron during any significant inflammation in our patients – ferritin rises because in fact as much iron as possible has been removed from the blood…because our infectious foes are fans of ferrous (not Ferris..😳)

Authors like Denic and colleagues contend therefore that “humans may have ‘failed to adapt’, genetically and culturally, to continuous deficiency of iron because relative iron deficiency was protective against many infectious diseases”.   So next time you find yourself cursing the ‘dragging feet deficiency’, perhaps rather than our Stone-Age genes we can blame the blinkin’ Agrarian ones! It’s food for thought.  Add to that, other major changes in the lives of women over time, like more time menstruating, both with a tendency to earlier menarche and significantly less cumulative time pregnant over our lifetime but I am sure there are many other theories out there.  Have you got one?

Where Could Should Would All the Iron Go?

I love a good iron question.  It makes me emit a sound like Jack Sparrow might, when faced with something pleasingly piratey 🏴‍☠️  Lucky because I am asked SO MANY GOOD ONES, SO OFTEN!!  Being one of the earliest minerals discovered to be essential…we know so much more about it than the other latecomers but that ABSOLUTELY DOES NOT MEAN WE KNOW IT ALL!  Sorry…I also tend to shout with excitement when faced with a good iron question. Yes, run for cover, I do have an even higher volume setting 🙉🙉

Just this month, I received this little ripper:
I was wondering about the expected timeframe for iron supplementation to improve ferritin levels?  

Yes of all the nutritional answers we can quickly ‘google scholar‘ our way towards, actual realistic expectations for response to treatment, is rarely an easy find! And yet, this critical clinical detail seems to be the thing that can leave so many of us stranded at sea with our patient prescriptions. Did I give up too soon? Or should it have worked already by now?  Some of the best naturopathic approaches are easy to execute…but when those patients come back after maybe a few repeats & perhaps minimal change you realise, you have a major piece of information MIA: what to expect.  Rapid relief or a cure by Christmas?!  Aka, it won’t happen overnight but it will happen.  So, great, let’s talk ironing out our expectations around responses to iron repletion!!!

First – We need know where iron goes First!

So if your iron deficient individual is actually suffering from iron deficiency anaemia, no guesses who the VIP (very important priority) is during repletion! Consequently, you can throw lots of iron at somebody (oral or even IV) and find there’s limited increase in their ferritin initially, a rather ‘disappointing’  show actually but if you keep your other eye (Jack Sparrow style) on their FBE, you’ll see the iron is being funnelled into producing haemoglobin and red blood cells. Sneaky! And if sensible can ever be associated with sexy…this is it! Ferritin is for iron surpluses only and right now, we ain’t got none!  Medical texts advise that in these scenarios, confirmation of efficacy equals an increase in Hb levels of approx 10g/L a week. In reality, no one wants to turn patients into pin cushions (esp when we’re trying to build iron back up not keep springing leaks!) so we might retest in a month or more. But an increase in Haemoglobin of 40g/L per month sounds rather aspirational with oral iron, doesn’t it!!

If we bring in the personalised perspective here, we recognise that most of our anaemic patients may only be just on the wrong side of the line, with values at 110g/L and tbh we would be shocked and a little worried if this grew in a month to 150 in a woman! Erythropoiesis is not the result of iron alone!  But the point is, if the iron replacement is working well enough, you should be moving out of anaemia within a month not a millennium!

And only once this job is done will the ferritin start to build.

So what if your patient isn’t anaemic – just low in ferritin? Well then, if you’ve a) fixed the leaks (unintended or excessive blood loss anywhere Rectal, Renal, Repro) and b) buoyed bioavailability (HCl & prebiotics while minimising iron-blocking issues like excess Ca, tannins etc) and c) corrected for low intake via a sound supplemental approach (daily dosing for those not consuming much dietary iron and alternate days for the rest) you should be seeing ferritin increase within the month of at least 10mcg/L, but hopefully more.  And if it’s not? Go back to the beginning of this little to-do list…because it means we’ve missed something. Doh!

Ironing Out Some Old Misunderstandings

It’s that time of year when we tend to set our intentions both personally and professionally.  For me, between the many meals, pressies and dunks in the river, I slip into some ‘silent work’. In particular, I find myself flagging a couple of key areas that I want to sharpen my knowledge in this year.  I’ve already picked mine…have you identified yours? 

For many practitioners if there is one topic in nutritional medicine that seems to be more generous than any other it would have to be iron: Iron gives us patients…loads of them! Patients who present with deficiency, with overload, with something in between but still noteworthy, or on iron and that’s causing them all sorts of problems. 

But Iron’s generosity doesn’t end there.

It also tends to give a lot of practitioners a bit of a headache!

That’s because a) we were mistakenly taught about iron as if it were just another one of the mineral mob and accordingly allocated grossly inadequate time to do more than scratch the surface of what we need to know and b) what we need to know, thanks to it being the most researched mineral, has undergone a couple of major revelations and revolutions since then anyway!   So we can benefit from Iron’s generosity most and leave its other unwanted pressies (the headaches, confusion, frustration & suboptimal management of patients) under the tree – we just need to give iron the real attention it deserves, filling in the gaps in ours and many people’s knowledge about this critical nutrient.  And boy, do we (and I mean everyone!! including doctors, midwives, pharmacists…anyone who has ever called iron deficiency on a client!!) need to learn how to correctly read iron studies!!!

Because iron also gives us much needed insight into other micronutrients and just how exquisitely sophisticated their roles & regulation can be. Thanks to it being one of the ‘older minerals’ we know more about it than any other and in turn we have the most advanced assessment methods: Iron studies, a collection of 4 parameters, like 4 chapters in a book or 4 key characters in a play, that need to be viewed separately and then together to understand the whole story.

Iron – Another Important Discovery

Yet another super-helpful part of Iron-Land has been mapped!!  Ever struggled to correct chronic iron deficiency in athletes or even just weekend warriors?  Yep, me too. One of the key barriers being the 2-3 fold rise in hepcidin in response to exercise. Hepcidin whose day job is an inflammatory signal that two-times as an iron uptake blocking agent at the small intestine.  In addition to other exercise-induced factors that either reduce Fe uptake or increase losses, it really is no surprise that these cases can be hard to treat. However, a recently published small Australian study has brought to light some constructive new information. Similar to the often talked about ‘anabolic window of opportunity’ whereby we encourage people to consume protein +/- CHOs within a short time-frame post-exercise to optimise exercise outcomes and negate negatives, these new findings imply the same might be true for optimal Iron uptake. But only in relation to exercise done in the morning! 

The key finding was when individuals consumed iron after 90mins of exercise in the morning they exhibited higher uptake than both when they took the iron at the same time but didn’t exercise beforehand or took it after exercising at night.

This is a game-changer for potentially ALL our patients who struggle with iron absorption.  With the key take-home being…not just take your iron preferably in the morning which we already know (when hepcidin is naturally lower as part of its diurnal rhythm) but before you pop that pill, pop on your sneakers and get busy sweating! How on earth might this be working?  Well this study demonstrated that while hepcidin rises after exercise typically for up to 6hrs…it is not yet ‘up’ and blocking within the first hour – gotcha! But why would this mean an even greater uptake compared with the same iron at the same time in the same individual…but a resting version of themselves?  Because exercise may in fact cause a transient leaky gut post exercise & enhanced nutrient uptake may be its silver lining!  A small study that actually punches above its weight, this one is worth the read – via a great comprehensive summary on Medscape if you have it or you can check out the abstract.

Our ever-expanding Iron knowledge gives us great hope for the improved understanding we are likely to reach with all nutrients in the future.  Let’s not forget Iron has about a 70 year head-start on other microminerals such as Zinc and almost a century on Selenium, which was identified to be essential in just 1979! 

And the contrast is apparent anywhere you care to compare and contrast the ‘older’ with the ‘younger’ nutrients. Just look at iron studies. A personalised detailed account of each individual’s iron story: how much you’re consuming, how effective you are at absorbing what you’ve been offered, how hungry that makes you for more and what good stores mean to you (not some fictitious average male or female)!  All told through 4 distinct but inter-related markers: serum iron, transferrin, transferrin saturation and ferritin.  What can we glean from our current routine assessment of Selenium in contrast?  Their short-term Se intake…yep. Looking forward to the multi-parameter markers of each individual nutrient we just might have at our fingertips in the future, thanks to iron nutrition which continues to teach us how sophisticated nutritional physiology really is 🙂

You Might Want to Write This Number Down

No you’re right, it’s not long enough to be a Hemsworth’s mobile number but actually it’s more sought after 😉 If you’re up to date with reading & recognising all the different patterns of Iron Studies & the stories they tell, which is a daily business for most of us, then you will know by heart the striking pattern we call, ‘Pseudo Iron Deficiency’. You know the one where your patient’s serum iron & transferrin saturation are mischievously trying to trick you into thinking you need to give this patient iron…when in fact this is absolutely not what they need! 

This is of course the result of the redistribution of iron during inflammation – iron is actively removed from the blood and  sequestered in the liver instead.  It’s designed to protect us from bacterial bogeymen, which is how our stone-age bodies interpret all inflammation of course. 

Doesn’t sound familiar? Ok you need to start here or even embrace a full overhaul of all things iron here.

But for those of you nodding so hard you’re at risk of doing yourself an injury, this number is for you.   We’ve often talked about the redistributional increase in patients’ ferritin levels in non-specific terms: it goes up..but by how much?  Of course we would like to know because no one is fooling us with this transiently inflated value…but can we make an estimation as to what this person’s ferritin will drop to once this inflammation is resolved? Yes.

This glorious magic number comes from Thurnham et al paper in 2010 who did the number crunching on over 30 studies involving almost 9,000 individuals to determine the mathematical relationship between inflammatory states & markers and the reciprocal increases in ferritin.  Their work is exceptional in that it also differentiates between incubation (pre-symptoms), early and late coalescence periods (if you want to differentiate your patients in this way and get even more specific then you need to read the paper), however, overall when we see a patient who has a CRP ≥5 mg /dL , we can multiply their ferritin by 0.67 and get a lot closer to the truth of their iron stores. Oh and another important detail they revealed, this magnitude of ferritin increase is more likely seen in women or those with baseline (non-inflamed) values < 100 ug/L..so generally more applicable to women than men. Thanks Thurnham and colleagues and the lovely Cheryl, my previous intern who brought this paper to my attention…you just took the guessing out of this extremely common clinical scenario 🙂

Is Integrative Medicine Mentoring On Your To-Do List?


Having just spent the last four days in Sydney delivering education at ACNEM to doctors, dentists, pharmacists and other medical professionals, I am reminded afresh about what we all share: a passion for problem solving and a diagnostic detective determination!  What’s truly wonderful is when  teaching shifts from didactic to dialogue-based, with sharing of our different training and expertise, as happened during this weekend. That’s when I find the gains are greatest.   Since the first time I was invited to speak at ACNEM in 2009 (that makes me feel very old!) I observed this collegiate and collaborative learning environment and, I realise in hindsight, worked on developing this a little more within my own naturopathic ranks via my group mentoring.

Over this decade my interactions with integrative doctors have grown in frequency and familiarity. Coming to see and appreciate ever more about important differences in the clinical context from mine and learning increasingly how I can help build a bigger stronger bridge for us to walk between the two approaches – in order to practice true integrative medicine.  With the best of both worlds.

What’s Presented in Each Session?

Each session is 1 hour & occurs monthly from January to November. Practitioners present their cases in a rotating fashion across the year.  The work-up of each case will include pathology interpretation, identification of other key clinical questions to be asked and specific additional assessments, drug-nutrient interaction review, discussion of differentials & a proposed integrative management model. Ideally (dependent on final group numbers) these case based sessions will be interspersed with several open Q&A sessions, during which Rachel can answer key questions you have in any given area & follow up on previously presented cases.

Group Mentoring…What’s it all about?

 

For all those Mentoring Virgins 😇 out there wanting a clearer understanding of what it’s really like to be part of my group mentoring, this video is a little snippet from a session with one of my groups. This year has flown by and I have thoroughly enjoyed working with each fabulous group of dedicated ‘life-long learners’.

OH YES!!…and the real announcement is…..(drum roll)…
It’s that time of the year….Applications open next week for GROUP MENTORING in 2020! 

As a result of the generous feedback and insights from our current Mentees, we are always fine tuning our program & level of service. Yep…it just keeps getting better and better every year!! We are keeping everything that so many practitioners have told us they love from the past 7 years (wow….have I been doing it for that long?!) and simply improving the already incredibly popular formula, with some great new features for 2020.

  • New 15min Follow up with one on one with me! via Zoom for those cases that have been presented in our group mentoring sessions.  This is a brand new format to follow up on how your client is going after the session – what’s working, and what’s happening now, what should you do next? Rachel will spend 15 mins with you on Zoom 1-2 months after you presented your client case.  The recording will then be uploaded to Basecamp so the whole group can catch up on the progress and extend our learning opportunities again.

    • New Graduate Groups –  great opportunity for New Grads to build confidence as they leap from student to practitioner, or for practitioners wanting to refresh their core clinical skills such as MindMaps, Pathology, Improved Case Taking etc.
    • Mental Health Primer Group –  topic based to build on your knowledge in the role of naturopathic medicine in Mental Health – from screening tools to key management issues, specialist diagnostics and beyond.
    • Mental Health General Group Mentoring – practitioners presenting their client cases with a focus on primarily Mental Health presentations.

Are Hormones Driving the Autoimmune Bus?

Back a few weeks ago I had the pleasure of presenting at the Integria Symposium and the even greater pleasure of listening to some of the fabulous speakers …you see I’ve heard my stuff before! 😉 The ‘Mosaic of Autoimmunity’ was delivered by the very funny and knowledgeable Professor Yehuda Shoenfeld, who reiterated the sequence of events now well recognised to precede and precipitate autoimmunity: genetic susceptibility + endocrine context + environmental trigger –>autoimmunity.

Clinicians know that overwhelmingly women dominate when it comes to autoimmune disease epidemiology and most understand that this is a consequence of oestrogen’s immunostimulatory effects.  Professor Shoenfeld, described the female, or E2 dominant, immune system as being ‘super charged’ and that increased rates of autoimmune diseases were a reflection of this. Sometimes practitioners do initially great work with a/immune clients – clearing up the diet & gut, ensuring vitamin D adequacy etc and then get ‘stuck’ or plateau with antibody levels that ‘won’t budge’.  Going back and checking the hormonal contribution in the case is often indicated. If the patient has an unhealthy E2 dominance and /or impaired detoxification and clearance of this hormone then working on this aspect often kickstarts the next stage of improvement.

A new thing to me (I know I’m a bit slow sometimes 🙁 )  was his mention of the potential link also with high prolactin (PRL).  The literature on this is extensive and hyperprolactinemia (HPRL), even just mild elevations, have been correlated with a very long list of both systemic and organ specific diseases including: (more…)

Nutritional Science & Debate in Action

 

Last weekend I attended the Science of Nutrition in Medicine Conference in Melbourne https://nutritionmedicine.org.au/enews/2015-02-06-Program.html and for those of you that didn’t make it I can tell you, it wasn’t your regular CPE event.  I’m not sure if the debate inside the presentations or outside in the breaks was more interesting but I can say that I haven’t seen this much stimulated thought, heated discussion and passion at company run events.  Why?
